ADMISSIONS | FORMS
In addition to submitting an online New Student Application, the following supplemental forms must be completed and submitted to the Admissions Office. All supplemental forms and records must be received in order for an application to be considered complete and family interview scheduled. We strongly recommend completed supplemental forms be faxed to the Admissions Office at (936) 890-5343. The fax number is located at the top of each form. Supplemental forms delayed or lost in the mail will also delay the completion of your application.
► New Student Application (one per applicant)
► Pastor Recomendation (one per family)
► Administrator/Counselor Recomendation (one per applicant)
► Teacher Recomendation (two per applicant, depending upon grade level)
► Records Release (one per applicant, depending upon number of previous schools attended)
