ADMISSIONS | FORMS
In addition to submitting an online New Student Application, a Records Release Form and an Information Release Form must be printed off, completed, signed, and submitted to the Admissions Office.
The Records Release and Information Release Forms must be received along with all records, recommendations, and assessments in order for an application to be considered complete. Upon completion, a family interview will be scheduled. We strongly recommend that all completed release forms be faxed to the Admissions Office at (936) 890-5343. The fax number can be found on each form. Forms delayed or lost in the mail will also delay the completion of your application.

► Records Release (one per applicant, depending upon number of previous schools attended)
► Information Release (one per applicant)
Applicants must also submit a completed Confidential Pastor Recommendation Form (one per family), Confidential Administrator/Counselor Recommendation Form, and two Confidential Teacher Recommendation Forms (depending upon grade level). However, the recommendation forms are electronic and are included within the online application. They will be emailed directly to individuals, designated by you, when the online application has been submitted.
